Sample Access Databases for Access 2003

Note that I am not an Access profession or developer. Access is just a hobby for me. I have no real world experience.

Survey Template -- This database is for creating surveys, survey questions, question answers, respondents and entering responses to survey questions.The database might also be used for questionnaires and for tests.

Generic Survey Tally – This database is for entering survey responses from paper survey forms.Names of survey respondents and the text of questions are not considered. It is just for getting the raw data from paper into a database.

Patient Assessments Based upon a newsgroup posting, this database is for entering patient responses to mostly yes-no-type questions. It could be modified for classroom tests or surveys.

Church Records Database Overview – This is an overview of a database I made for my own use to transcribe church records. This is the project that got me into using Microsoft Access.

Anzeiger Newspaper Article Overview – This is also a database I made for my own use. The task was to transcribe and translate obituaries from the Tell City Indiana Anzeiger.

Subform Example – An example form with two subforms.

Dealer Chemical Transfers – A subform example based upon a newsgroup posting. Tracks quantities on-hand by dealer, and borrowing and lending between dealers.

Calendar Table – Some examples using a calendar table to compute work days.

Patient Appointments – Based upon a newsgroup posting, this database is for scheduling patient appointments. It is just an exercise, and I cannot say whether it is the correct approach for this kind of problem.

Student Attendance – This example is based upon a newsgroup question.
